The main process of the accident

A worker in a tool workshop of a factory led his apprentice to use a butt welding machine to weld tools. When the master was installing the welded parts on the electrode clamp and aligning the position, he verbally instructed the apprentice to support the tail of the workpiece. When the upper arm of the pneumatic clamp was pressed down, the apprentice had not yet pulled out his right index finger and it was squeezed between the tail of the workpiece and the shim, causing the end of the index finger to break and open fracture.

The main reasons for the accident

(1) The two people were not coordinated in their work. When the master pressed down the pneumatic electrode clamp, he did not remind the apprentice to pay attention, nor did he get the apprentice's response;

(2) When supporting the workpiece, the apprentice held the shim incorrectly and should not hold the shim up and down with his fingers.

Main preventive measures

(1) The two people should work closely together, agree on verbal contact information in advance, and proceed after receiving the signal.

(2) Special tools should be made to clamp the shim to avoid fingers getting involved in the clamp jaws.

(3) Improve the process performance of the welding machine fixture so that it can be deflected and moved vertically and horizontally.

(4) Strengthen safety technology learning and provide safe production skills.
