The main process of the accident

Gu, a welder at a shipyard, applied to the on-board firefighters for permission to start a fire, but the firefighters approved the application before they arrived at the scene. After Gu cut the wire with gas, the oil on the bottom of the ship was splashed with sparks, igniting a raging fire. The on-site personnel failed to put out the fire with water and fire extinguishing agents, resulting in 5 deaths, 1 serious injury, and 3 minor injuries.

The main reasons for the accident are:

(1) The firefighters neglected their duties and blindly approved the application before they arrived at the scene.

(2) There was oil under the fire site;

(3) The on-site personnel lacked fire-fighting knowledge.

How to prevent the accident?

(1) After receiving the application for starting a fire, the firefighters should go to the scene to inspect and confirm that it is safe before issuing a fire permit;

(2) The oil under the fire site should be clearly visible;

(3) The safety knowledge of employees should be strengthened.
