Direct cause of the accident
When the company was repairing the steam leak at the joint of the steam jacket and the material pipeline of the discharge pipe from the acidic 2,4-dinitrochlorobenzene separator to the water washing pot in the dinitrate workshop, the hot work personnel violated the regulations of electric welding without obtaining the welder operation qualification certificate and without taking safety measures such as disconnecting the pipeline, cleaning, and replacing, resulting in the acidic 2,4-dinitrochlorobenzene in the pipeline being decomposed and exploded by strong heat.
