A welder suffered an electric shock accident while welding.

Main sequence of events in the accident:

A welder at a factory was performing welding operations at a temporary outdoor construction site. During the operation, he found that there was no power distribution box for connecting the welding machine. He then proceeded to strip the insulation from the ends of each wire of the cable and connected them directly to the exposed power lines. However, he mistakenly connected the neutral wire to the live wire. When he adjusted the welding current and touched the machine's casing, he was electrocuted, resulting in his death.

Main causes of the accident:

The welder was unfamiliar with relevant electrical safety knowledge and mistakenly connected the neutral wire to the live wire, causing the welding machine's casing to become live, leading to electrocution and death.

How to prevent such accidents?

Wiring work must be performed by a qualified electrician; welders are not permitted to perform this work themselves.
