Professional fire extinguishing equipment during welding

A welder named Gu from a shipyard applied for a hot work permit from the on-site firefighter. However, the firefighter approved the permit before even arriving at the scene. After Gu used oxy-acetylene to cut a flammable wire, sparks flew from the oil slick on the ship's hull, igniting a massive fire. Those present tried to extinguish the fire with water and fire extinguishers but were unsuccessful, resulting in 5 deaths, 1 serious injury, and 3 minor injuries.

The main causes of the accident were:

How can this accident be prevented?

(3) Employees must receive enhanced safety training.
