The main course of the accident:
A welder at a factory was welding at a temporary outdoor construction site. While working, he discovered that there was no power switch for the welding machine. He then removed the electrical seal from the ends of each conductor on the cable and connected them to the outdoor power grid. However, he mistakenly connected the neutral wire to the live wire. When he adjusted the welding current and touched the outer casing with his hand, he suffered an electric shock, resulting in his death.
The main cause of the accident:
The welder was unfamiliar with electrical safety and mistakenly connected the neutral wire to the live wire, causing the welding machine casing to become live and resulting in his death from electrocution.
How can accidents be prevented?
Wiring work must be performed by an electrician; welders are not allowed to do it without authorization.
